Everything SMART Health IT operates is free and open. It is a standards organization (Boston Children's Hospital / Harvard Medical School), not a commercial API vendor - there are no paid tiers, no API keys to buy, and no sales motion. The specifications (SMART App Launch, FHIR Bulk Data, SMART Health Cards) are open HL7/community standards, the client libraries and reference servers are open source under the smart-on-fhir GitHub organization, and the public sandboxes (open FHIR R2/R3/R4 servers, the SMART App Launcher, and the reference Bulk Data server) are free to use with no registration. The sandboxes serve synthetic data only and are intended for development and testing, not production workloads or real PHI.
